My doctor wants me to try acupuncture for my back pain. Will Medicare cover any of this?

Will Medicare cover acupuncture? Yes, Medicare will cover acupuncture, but it's only for chronic lower back pain. So, keep that in mind. You can't get Medicare to pay for acupuncture if you have other issues beyond that. Now, some Medicare Advantage plans will also cover acupuncture treatments, but it's usually for the same condition, meaning chronic lower back pain. So check with your agent, who will have access to a variety of summaries of benefits for the different plans, and find out before you sign up.

Original Medicare (Part B) only covers acupuncture for chronic low back pain, and it has to meet all of these criteria:
• The pain has lasted 12 weeks or longer
• It’s not associated with surgery, pregnancy, infection, or cancer
• The acupuncture is provided by a qualified practitioner (MD, DO, NP, PA, or an auxiliary provider under their supervision)
If you qualify, Medicare will cover:
• Up to 12 sessions in 90 days
• An additional 8 sessions if you’re improving - 20 visits max per year
What Medicare does not cover:
• Acupuncture for neck pain, migraines, arthritis, sciatica, or general pain
• “Wellness” or maintenance acupuncture
• Most stand-alone acupuncture clinics unless they meet Medicare’s provider rules
Also important: Medicare Advantage plans may offer broader acupuncture benefits, sometimes covering more conditions or visits than Original Medicare.
Bottom line:
If this is chronic low back pain, there’s a good chance some of it is covered. If it’s for anything else, you should assume Medicare won’t pay unless you’re on a Medicare Advantage plan with extra benefits.

You can receive up to 12 treatments in 90 days, with the potential for 8 more sessions for a total of 20 per year, provided your doctor confirms you are improving.
The treatment must be administered by a qualified, licensed practitioner, and you are responsible for the Part B deductible and 20% coinsurance for approved services.
If you are covered by a Medicare Supplement plan, the 20% coinsurance may be covered depending on what plan you are in. If you are covered by a Medicare Advantage plan, a prior authorization may be required and you will more than likely have a co-pay.

Here’s how it works:
Medicare will cover up to 12 visits to try and help with the pain.
If it’s helping, they’ll cover 8 more, for a total of 20 visits a year.
If it’s not helping, they stop covering it and you’d have to pay yourself.
You also need to see someone Medicare approves — like a doctor or nurse with special acupuncture training (not just anyone with an acupuncture license).
After you pay your Part B deductible (which is $257 in 2025), you’ll pay 20% of the cost for each visit.
Important: They only cover it for low back pain — not for other things like neck pain, arthritis, or stress.
Some Advantage plans have it included within their plan so be sure to check with your plan if you are on an Advantage plan.

Medicare Part B (Medical Insurance) only covers acupuncture (including dry needling) for chronic low back pain defined as:
Lasting 12 weeks or longer
Having no known cause (for example, it's not related to cancer that has spread, or an inflammation or infectious disease)
Pain that isn't associated with surgery or pregnancy
Medicare covers up to 12 acupuncture treatments in 90 days for chronic low back pain. If you show improvement, Medicare covers an additional 8 sessions (for a maximum of 20 acupuncture treatments in a 12-month period). If you aren't showing improvement, Medicare won't cover your additional treatments and you’ll pay 100% of the costs if you continue getting them.
Some individual Medicare Advantage and Supplement plans also offer Routine Acupuncture.
