Your provider should be able to tell you if any procedure is covered by Medicare. If you are having trouble finding out from the provider, you can search the Medicare Cost Estimator site.

I have had to contact Medicare to find this out myself. Medicare will only answer "If it's medically necessary." Unless it is something specifically mentioned in Medicare's coverage, it depends on it being medically necessary. The doctor or provider scheduling should know if it will be covered as well. There are some disclaimers though of things not covered such as an experimental treatment.

You can find out if a particular procedure is covered by Medicare by obtaining the name of the procedure and the billing code and then either calling 1-800-Medicare or go to Medicare.gov and using the Medicare coverage tool.

It’s worth calling the provider’s billing office first because they deal with the exact procedure codes that get sent to Medicare or your plan. In some cases, they’ll need to request prior authorization anyway, so they can flag any issues upfront. Your agent can help you understand your coverage overall, but the billing department usually has the clearest view of how that specific service will be billed.

The best option is always contact your agent and ask for your plan's summary of benefits book, there you can find the details of what your plan will cover and how much copayments, deductibles, and/or coinsurance ( if any) you will be responsible for.

You can ask Medicare at 1-800-Medicare, go online and use their procedure price look up tool or contact your provider to see if it's medically necessary. If you have a Medicare plan already you can use the EOC evidence of coverage and/or call the carrier to inquire. Definitely reach out to your doctor and their office (speak with the billing dept).
