No referral is needed for a specialist if you are on a PPO however if you are seeing a specialist outside of the network you should expect to pay more than one inside the network.

NO, they do not. However, not all providers accept PPO. It's not guaranteed acceptance. PPO does provide somw flexibility to move in and out of network
