Yes, there are Medicare plans designed to provide additional support for people with certain chronic health conditions. These are called Chronic Condition Special Needs Plans (C-SNPs). If you have a qualifying condition—such as diabetes, chronic heart disease, or other eligible chronic illnesses—you may be able to enroll in a plan that offers benefits and care coordination.

Keep in mind that having a family history of a condition alone does not qualify you. You must have a diagnosis of an eligible chronic condition to enroll in a C-SNP.

If you have Original Medicare Part A & Part B, normally Medicare Part B will cover these conditions at a Doctor whom accepts Medicare assignment. If Medicare approves the Part B claim or claims then a Medicare Supplement insurance Plan G or a Plan N will be what you want with secondary insurance coverage. Medicare Supplement insurance Plan G or a Plan N will be your secondary insurance on approved claims on Medicare Part A & Part B. It is also good while traveling. Thank you.

There are Medicare Advantage plans that are for chronic conditions. However, in most states Medicare beneficiaries can only enroll in these plans if they have heart issues or diabetes.
