Which states have 5-star Medicare Advantage plans available?

Answered by 6 licensed agents

For 2026, 5-star Medicare Advantage plans are available in 19 states plus Puerto Rico, although availability is county-specific.

Based on the current 2026 plan data, the states include:

California, Colorado, Florida, Georgia, Illinois, Iowa, Kansas, Maryland, Massachusetts, Michigan, Missouri, Nevada, New Jersey, New York, North Carolina, Pennsylvania, South Carolina, Texas, and Virginia — plus Puerto Rico.

Having a 5-star contract somewhere in a state doesn’t mean every beneficiary in that state has access to it. It has to be offered in the beneficiary’s county/service area. If it is, the 5-Star SEP can generally be used once between December 8 and November 30 to move into that 5-star plan.

For the 2026 plan year, 5-star Medicare Advantage plans are available in at least part of the following states:

Colorado, Florida, Georgia, Illinois, Kansas, Maryland, Massachusetts, Michigan, Missouri, Nevada, New Jersey, New York, North Carolina, Pennsylvania, South Carolina, Tennessee, Texas, and Virginia.

Puerto Rico also has 5-star Medicare Advantage plans available.

One important distinction is that a state having a 5-star Medicare Advantage contract does not mean a 5-star plan is available in every county. Medicare Advantage plans operate within specific service areas, so availability depends on where you live.

For **2026**, CMS lists **18 Medicare Advantage contracts that earned the highest 5-star rating**. ([CMS][1])

The states and territory with **5-star Medicare Advantage plans available** include:

* **Florida**

* **Georgia**

* **Illinois**

* **Missouri**

* **Nevada**

* **New York**

* **North Carolina**

* **Texas**

* **Puerto Rico**

There is an important catch: **a 5-star plan may only be available in certain counties**, not throughout the entire state. For example, the 5-star Anthem plan is available in selected New York counties, while 5-star plans from UnitedHealthcare and others have specific county service areas. ([Medicare.org][2])

### ⭐ Why the 5-star rating matters

A Medicare Advantage plan with **5 stars is CMS's highest quality rating**. Also, people enrolled in Medicare have a special opportunity to enroll in a **5-star Medicare Advantage plan outside the normal enrollment periods**, if one is available in their area and they meet the eligibility rules.

**If you're asking because you're comparing plans for someone in Florida:** Florida is one of the states with 5-star plans in 2026, including **Devoted Health Plan of Florida** and **Leon Health**. ([CMS][1])

For 2026, 5-star Medicare Advantage plans are available in select areas of several states. Availability is county/service-area specific, so a 5-star plan may not be available everywhere within a state.

CMS reports 18 MA-PD contracts received an overall 5-star rating for 2026.

Many states have Medicare Advantage plans rated 5 stars for 2026, but availability depends on your county and ZIP code. In Michigan, most highly rated plans have 4 or 4.5 stars, and not all plans are available statewide.
