The newest change for 2025 is No Donut Hole in the Medicare Part D patient maximum. The new limit for out of pocket prescription costs are $2000 limit. Basically, is the medication you require is covered by your Part D plan or MAPD, the new guidelines limit your maximum costs per year. Additionally, there is a new monthly payment plan option to cover high cost Brand Name medications.

Oh, without a doubt, the elimination of the donut hole for prescription drugs was a game changer. And the out-of-pocket maximum on covered prescriptions going to $2,000 a year was a lifesaver for some people that were on very expensive medications. It did mean an increase in deductibles. However, I see a big push toward making prescriptions more affordable, and that's only a good thing for seniors.

I would say the biggest trend that is having a positive impace on medicare affordability is the increase in prescription drug coverage. The out of pocket maximum dropped significantly and the elimination of the doughnut hole of catastrophic coverage has saved seniors thousands.

I think one trend that is making a big difference is that people in general are paying attention and taking better care of their health overall by doing preventive screening tests and choosing a primary care physician to help manage ongoing conditions. There is better awareness of how healthy living activities including daily movement, health food choices and adequate sleep and taking care of your mental health will improve your overall health.

The fact that CMS has FINALLY held PBMs’ feet to the fire and negotiated prices on some of the most outlandishly expensive and frequently used medications is potentially a major game changer. It’s only 10 meds for 2026 but there’s more on the way in coming years. Fingers crossed that it will begin to shift the power back to the Medicare patient to get the Rx care they need and not be priced out.

Once you reach this maximum amount out of pocket, you have no further copayments or coinsurance for the remainder of the year.

Newly introduced in 2025 was the Medicare Prescription Payment Plan. Instead of paying for prescriptions in full at the pharmacy, a beneficiary may choose to receive a monthly bill from their drug plan. This allows individuals to spread their total prescription costs evenly across the year, making budgeting easier. The MPP is available to anyone with a Medicare prescription drug plan or a Medicare Advantage plan that includes drug coverage.

The MPP greatly helps those beneficiaries who face high drug costs, particularly at the beginning of the year. The MPP has no additional fees, and participation in the MPP is free. There are no hidden costs associated with spreading out payments.
