Your provider should have sent your Annual Notice of Change (ANOC) by September 30 each year to inform you of changes to your coverage, costs, and more for the following January. You should receive it in the mail or by email, depending on your plan's preference.
If you have not received it yet, contact your insurance provider for a replacement copy.

Here's the short answer:
Your plan provider is required to send your ANOC by September 30, 2025.
This notice outlines any changes to your coverage, costs, or network that will take effect on January 1, 2026. Even if you’re happy with your plan, it’s really important to review this document. Things like premiums, copays, or which drugs are covered can change from year to year.
What should you do when you get it?
Look it over carefully. Pay special attention to any changes in premiums, out-of-pocket costs, covered services, and your provider network.
Compare your options during Medicare Open Enrollment (October 15–December 7, 2025), in case another plan may better suit your needs for the upcoming year.

It is very important to read them, especially if your current Plan might be one that is being terminated.
One important note is that if you have a Medicare Advantage plan that is being terminated,
PLEASE hold on to this letter, as this letter will give you Guaranteed Issue for a Medicare Supplemental Plan....... WITHOUT MEDICAL UNDERWRITING! That could possibly greatly save you money, especially if you have current or pending medical issues.
These ANOC letters also state the Premium cost and benefit changes in your current Plan going forward. This can help you to investigate a new Plan, if you would like to.

If you don't receive it, you can call and request one from your insurer. Look on the back of your card, and the first number listed will be for member services or customer service. The easiest way to get it is from your online portal if you have created an online account, or from your insurance broker (if you use one).
It is very important to review the ANOC, so try to get it soon. You only have until Dec 7, typically, to make changes to your plan.

If you do not get this notice, contact your plan.
Just to add piece of mind, if you have not received this notice by Sept 30, no worries.
You still have time to contact them the get it sent to you to review in plenty of time to make changes to the plan if you choose to do so. Changes will go into affect January 1 - of the following year.
You can also consider signing up for this information electronically, if your plan offers that.

Plan providers need to have their next year's changes in place before they mail out the ANOC and the goal is to have it in the hands of the member before the Annual Enrollment Period (AEP) begins on October 15th of each year.
