Q&A with David Alelishvili

What's the most important question I should be asking about Medicare that I probably haven't thought of yet?

Answer: One of the most important questions you might not have thought to ask is: 'What happens to my Medicare coverage if my health needs change over time?"

Many people focus on what Medicare covers when they first enroll, but it's crucial to understand how your coverage might need to adjust as your health needs evolve. For example, Medicare offers different parts (A, B, C, D), and over time, you might find that you need additional coverage, like supplemental insurance (Medigap) or a different plan that better aligns with your needs. Additionally, asking about the flexibility of your plan, how it handles new treatments, specialist care, or prescription drugs, can save you from gaps in coverage or unexpected costs down the road. Knowing these details ensures that you’re not just covered today, but also set up for the future, as your healthcare needs change

What's the trade-off between a Medicare Advantage PPO and HMO when it comes to flexibility?

Answer: With a PPO, you have more flexibility to see any doctor or specialist, even outside the plan's network, without requiring a referral. This is ideal if you prefer to have the freedom to see different providers or need to travel outside your local area for care. However, this increased flexibility often comes with higher premiums, higher out-of-pocket costs, and potentially higher co-pays for out-of-network care.

On the other hand, an HMO typically requires you to choose a primary care physician (PCP) and get a referral to see a specialist. This structure limits your provider options to those in-network, but it tends to offer lower premiums, lower out-of-pocket costs, and a more streamlined care coordination process. The trade-off here is that while you save on costs, you sacrifice some freedom in choosing providers and have less flexibility, particularly if you need to see a specialist or seek care outside of the network.

In summary, if you value flexibility and are willing to pay a little more for it, a PPO may be the right choice. But if you prefer lower costs and don't mind the restrictions on provider choice, an HMO might be a better fit.

I'm healthy and barely use my coverage, but I'm still paying more every year. Why isn't there a discount for people like me?

Answer: I completely understand how frustrating it feels to stay healthy, barely use your coverage, and still watch your costs go up year after year. As a licensed Medicare broker in New York, I can tell you that while Medicare doesn’t offer direct discounts for being healthy, there are quite a few programs available—especially in New York—that can help reduce your out-of-pocket costs. Many of them focus on lowering prescription (Part D) expenses, which can still make a big difference.

Whether you have a Medicare Supplement plan or a Medicare Advantage plan, it’s extremely important to work with a broker who takes the time to do a thorough comparison. I meet with clients every day who think they have a great plan—until we sit down and go over the details. That’s when the surprises start. Often, they’re shocked by what their current broker never told them, or just plain confused about the fine print.

If you have a few minutes, I’d love to do a no-cost, no-obligation comparison for you. It won’t cost you anything, and it might just save you a lot.
