My friend lives in a different city and has a much more detailed Medicare plan. Is their plan dependent on their location?
Answer: Yes! Driven by primary address. State related! Every state is different as Medicare is federal and insurance governed by states.

My Medicare Advantage plan listed my doctor, but now they say he's out of network. How is that even allowed?
Answer: MCO are advantage plans which is managed care organizations. This is to prevent fraud. Guessing you took an HMO plan. Must use network doctors or balance billed for PPO plans. HMO is health maintenance organizations which started in the 80’s. MCO’s are to prevent fraud waste and abuse.
Will Medicare cover the cost of Medicare treatment?
Answer: If expense is approved coverage under Medicare will be invoiced but the individual medigap or advantage plan picks up based on Medicare approved expense. Typically Medicare part B is 80/20 so there are costs involved in addition to current part b monthly premium.
