I start Medicare in 2026 and a one-time 401(k) withdrawal in 2024 placed me in the second IRMAA tier for part of that year. When should I file Form SSA-44 so my 2027 premium returns to the lowest tier?

Answer: Medicare IRMAA is based on your tax return from 2 years prior so,

2024 income, affects 2026 premiums

2025 income, affects 2027 premiums

Since your one-time 401(k) withdrawal happened in 2024, it raises your 2026 premiums (IRMAA applies). It does NOT affect 2027, unless 2025 income is also high. You should file Form SSA-44 (if applicable). After you receive your 2027 Medicare premium notice (usually November–December 2026) or anytime in 2026 if you already know your 2025 income is lower.

Can I switch from a Medicare Advantage plan to a Supplemental/Medigap plan during the Annual Enrollment Period without answering health questions?

Answer: No, switching from a MAPD plan to a Medigap plan during the AEP does automatically qualify you for guaranteed issue enrollment without being underwritten. You may switch to Original Medicare without answering underwriting questions but not a Medigap plan.

Will my Medicare plan work when traveling to Europe?

Answer: Unfortunately, Medicare does not cover services provided outside the US. Since Europe is outside the US, you will be required to pay for 100% of your care.

Are Medicare Advantage plans really "free," or is that just clever marketing?

Answer: Medicare Advantage plans are absolutely not free. Medicare part A, hospital is free for those who worked a full time job and paid into social security and earned 40 quarter credits. If you did not earn enough quarter credits and wish to have part A, you will be required to pay a premium. Medicare part B Medical has a premium for all beneficiaries, and this year the premium is $185. You can get help pay this from the Medicare Savings Program, if your income qualifies you. Also, Medicare Advantage has Copays, deductibles and coinsurance cost sharing responsibilities for all beneficiaries that are does not have a full level of Medicaid.

Answer: I absolutely love when I go into a home and assist a beneficiary with his/her Medicare plan, and see the relief I bring to them after I explain how Medicare works for them.
