Answer: This is a great question. Here is a summary of how it works:

Extra lifetime reserve hospital days are a pool of 60 additional inpatient hospital days that Original Medicare provides over your entire life once a single hospital stay (benefit period) lasts longer than 90 days.

How Lifetime Reserve Days Work Under Original Medicare (without a Medicare Supplement plan)

• Days 1–90: Original Medicare covers your first 90 days of inpatient hospital care per benefit period (subject to your Part A deductible and daily coinsurance after day 60).

• Days 91–150: If you stay past 90 days, you can tap into your 60 lifetime reserve days.

• Coinsurance Cost: You pay a high daily coinsurance rate out-of-pocket for each reserve day used ($868 per day in 2026).

• Lifetime Limit: You only get 60 of these reserve days total in your life. Once they are used up, they do not renew.

• After Day 150: If you are still in the hospital after your 60 reserve days are gone, you pay 100% of all hospital costs.

What Medicare Supplement (Medigap) Plans Do

Medicare Supplement policies work alongside Original Medicare to help cover these high out-of-pocket costs:

• Covers the Coinsurance: Standardized Medigap plans pay the daily coinsurance for lifetime reserve days (such as the $868 daily cost in 2026). (Note: Plans K and L cover a percentage rather than the full amount).

• Adds 365 Extra Days: Once you completely run out of your 60 lifetime reserve days, Medigap plans provide coverage for up to 365 additional inpatient hospital days during your lifetime.

In other words- a Medicare Supplement plan will cover that $868/day cost if you have plan F,G, or N and you stay longer than 90 days. These plans will cover you up to 365 extra days if needed (during your lifetime).
