What costs count toward the annual out-of-pocket maximum on a Medicare Advantage plan?

Answer: The Max Out Of Pocket (MOOP) consists of the member's financial contributions toward copays (fixed dollar amounts for services), coinsurances (percentages of allowable charges) and the plan medical deductible.

It is important to not that prescription drug coverage has a seperate MOOP each year and that amout is set and regulated by CMS each plan year.

Plan premiums, dental and vision costs above the plans fixed allowances, balance billing, part B excess charges and services not covered by the plan are excluded from the plans annual out of pocket maximum and may be the responsibilty of the member.

Does Medicare cover Symbicort?

Answer: Symbicort is typically prescribed as a metered-dose inhaler. This is not covered by Original Medicare. It is most often covered by a Medicare Part D prescription drug plan.

However, if the appropriate criteria are met, Medicare Part B may cover the active ingredients budesonide and formoterol respectively when they are prescribed as a nebulized solution for use with an at home nebulizer. It is important that the nebulizer is medically necessary and prescribed as durable medical equipment (DME).

Can I get SilverSneakers with Original Medicare?

Answer: Short answer: no. Original medicare does not include silver sneakers or fitness programs. Some insurance companies include fitness programs with medicare supplemental insurances (medigap) and fitness programs are very common in Medicare Advantage plans. See your local agent for more details.

Do both spouses need to submit Form CMS-L564 to prove coverage through one spouse’s employer?

Answer: Yes, both should submit the form if they are both applying for Medicare. With the length of time it can take for Mediare to process enrollments, I wouldn't try to shortcut the process.
