Answer: Always ask your agent what a plan covers. Original medicare does not cover Dental, Hearing or Vision. Most MAPD plans cover D/V/H which is outlined in bth the summary of benefits and Evidence of Coverage.

Answer: HMO's are designed to manage care, minimize costs and is in-network only. To see a spec outside an hmo, you'd need a referral. Travel outside the state, emergence care and urgent care is available and if doctor accepts carrier,it's considered in-network.

HMO-POS is designed the same as an HMO but you see a specialist and co-pays should be same as in-network.

PPO's are set up for the beneficiary to see anyone w/o prior authorization or referrals. Good for those who travel. generally, co-pays are slightly higher and can contain coinsurance if out of network
